​Community Action Plans

Community Action Plans are developed as part of the second phase of the 2013 Mecklenburg County Community Health Assessment (CHA). The goal is to create an actionable plan to address the top four issues identified in the CHA priority setting process. These issues are:

  1. Chronic Disease
  2. Mental Health
  3. Access to Care
  4. Violence
