The HIV Case Management Program provides services for people living with HIV/AIDS of all ages with support services needed to maintain the medical stability of the clients and assure continuity of care.

Provides a range of client-centerd services that include case management. We link clients with healthcare, and psychosocial services to insure timely, coordinated access to medically appropriate levels of health.

Client Eligibility

Assist people of all ages living in Mecklenburg County with HIV/AIDS of all ages.
