Documentation of Hardship for SWIM Mitigation (This Form applies to property developed after the SWIM Ordinance took effect in 1999.  This completed form must accompany the completed Application Form to Impact SWIM Stream Buffers.  See Chapter 4 of SWIM Implementation Guidelines for details.)
