In the past, Mecklenburg EMS Agency has been a primary supplier of Do Not Resuscitate, or DNR forms for those in Mecklenburg County wishing to receive them.
Beginning April 1, 2002 the new Portable Do Not Resuscitate Form, designed for general use by any healthcare agency or professional, became available through the Office of Emergency Medical Services.
