Enrollment Process for MeckCARES and National Evaluation

The family/legal guardian calls MeckLINK at (704) 336-6404. MeckLINK screens via telephone and if youth meets criteria for medical necessity, the family chooses a community support provider for diagnostic assessment and care coordination. Or,

The Community Support provider informs the TA Coordinator of possible MeckCARES enrollee. TA Coordinator can be reached at (704) 432-4593.

The diagnostic assessment is completed w/in 30 days of initial community support appointment. The Community Support provider begins process to develop the person centered plan.

The Community Support provider works with the family to assemble a Child and Family Team and begin the Wraparound process. Additional services are identified as needed.

The family is provided with written materials and consent forms for contact by ParentVOICE and UNCC evaluation staff.

If the family agrees to participate in MeckCARES, the TA Coordinator works with Community Support provider to assign a System of Care coach and a case consultation team. A family may choose not to participate in MeckCARES.

The TA Coordinator, Community Support provider, UNCC evaluation team and the Parent Advocate invite the family/guardian to participate in the National Evaluation.

If the family agrees to participate in the National Evaluation, then a consent form is signed.
