All rebates are made payable to the individual/entity that retains ownership at the time the Certificate of Occupancy is issued. Please make sure all submitted paperwork is completed as such. General Contractors should not submit forms, unless doing so with the express permission of the owner, and with the owner's completed W9.
Green Permit Rebate Application
For projects that are broken down into more than one permit, please list the permit numbers that should be considered for each application. Keep in mind that rebates will follow the certification received, for example, if only a part of your project receives LEED certification, then only list the permits that are applicable. Only one application per project is required.
