In order to request access to CAREWare, agency's must submit an agency agreement (one-time) and we must have an user account agreement on file for each user.

- CAREWare Agency Agreement
Each agency must have an agreement on file with the Ryan White Program Office. This agreement ensures that proper measures are in place at your agency to ensure proper dissemination and logging of information available through the CAREWare and that your users and other staff and employees will use, and disseminate information available through CAREWare only when authorized and necessary.
- CAREWare User Account Request
Please submit one account request form for each user requesting access to CAREWare.
