Dear Colleagues,
 
We are pleased to announce that as of today, November 4, 2008, we are again partnering with Charlotte Dental Associates for provisional dental services to Ryan White clients.   These services are extremely beneficial and will lead to improved health outcomes for individuals living with HIV/AIDS. 

Charlotte Dental Associates will be accepting client referrals from all Ryan White Providers on a first come first serve monthly basis. Please note that a limited number of appointments are available each month.

When making a referral for a dental appointment, complete a certified referral form in its entirety. Procedure/Instructions are also available on our website at www.meckhealth.org/RyanWhite.htm.
Please note that fiscal responsibility for incomplete Certified Referral Forms rests with the referring agency.  If you have any questions please contact your Contract Coordinator.
