Community Health Assessment
Our Priority Health Areas (as identified in our 2007 community priority setting meeting):
  1. Preventing Chronic Disease
    (through improved nutrition, increased physical activity, and tobacco cessation)
  2. Access to Healthcare
  3. Mental Health
  4. Environmental Health
    (Healthy places supporting healthy choices)
  5. Substance Abuse Prevention
  6. Injury Prevention
  7. Responsible Sexual Behavior
  8. Maternal and Child Health
Community

The Community Health Assessment is conducted every four years; the most recent assessment was conducted in February 2007. This information is submitted to the state and is component of County Health Department Accreditation.

This assessment was completed by hosting a community health priority setting meeting.  Individuals representing a wide range of community agencies/organizations attended this meeting and were presented with data collected from various sources. 

Mecklenburg Healthy Carolinians uses this information to guide our efforts to support, promote, and encourage partner activities and collaboration.
