Our Vision for the Community…
- People are physically, mentally, and emotionally healthy.
- The community has a collective "consciousness about health" and health is high on the community's list of values.
- Everybody has access to quality health care and information.
- Every child has a medical home and access to appropriate care.
- Our air and water are clean.
- Injuries are not accidents; they are prevented.
- Policies support a safe, walkable, pedestrian community with opportunities for good nutrition (e.g., grocery stores, organic gardens, affordable healthy foods) in all neighborhoods.
- A sense of community is restored; neighbors look out for neighbors and people support each other.
- Cultural and spiritual beliefs and value systems are consistent with a high value on health.
- Families are supported and the family unit is preserved (by employers, policies, etc.).
- Relationships between families and practitioners are strong.
- People trust community institutions, organizations, and professionals.
- People partnering with health care providers and employers to share responsibility for health care.
- Schools partner with parents to educate children about health and promote healthy lifestyles.
- Health language is appropriate given a family's background, culture, and the meaning in the message; people leave health practitioners understanding what they said and what it means for them.
- Disparities in health outcomes no longer exist among groups and populations in our community.
- Access to and delivery of health care is free of stigma (e.g., based on race, income, substance abuse, mental health).
- People feel free (i.e., safe) to recover, and community supports are there to help them.
- Government, organizations, institutions, and the community work together to sustain health.
