HIV Case Management

Red Ribbons

The goal of the HIV/AIDS Case Management Program is to improve the quality of care for qualified clients who are HIV positive or have been diagnosed with AIDS, through the coordination of needed services. For more information on case management or to make a referral, please call (704) 336-2818 or (704) 591-1332.

Who is eligible?

  • Documentation of a positive HIV status or a diagnosis of AIDS;
  • A potential client is one that has Medicaid or one that may not be covered by any type of medical insurance.

Services Include:

  • Assesses the client’s needs
  • Advocates for the client and offers support.
  • Helps client receive needed resources, such as:
    • Hospital care
    • Personal care services
    • Legal assistance
    • Home care
    • Transportation
    • One-one-one personal counseling
    • Support groups
    • Education/information
    • Hospice services
    • Doctor/dental visits
    • Mental health counseling
    • Visiting nurses
    • Spiritual support
    • Financial assistance
    • Food
    • Housing

Costs:

  • We bill Medicaid or assess Ryan White Funds.  There are no fees billed to the client.

What you need to bring on your assessment appointment:

  1. Photo ID or proof of residence
  2. Proof of HIV/AIDS status (May have to be retested if this information is not available to be retrieved or you will need to sign a release of information to get this information from testing facility or Medical Doctor.)
  3. Social Security Card or INS paperwork, or if undocumented bring a letter stating such or write letter yourself and have case manager co-sign.
  4. W6 form, Pay stub or SSDI or SSI award letter or any other type of proof of income.
  5. Any other information you may feel that is pertaining to your care/needs.

If you have transportation issues a bus pass can be sent to you or other transportation arrangements can be made to bring you to the Health Department or another site for your appointment. The intake/admission process can be lengthy so be prepared to stay for a few hours.  If you have children you will need to arrange childcare. If for any reason you are delayed for your appointment, please notify the case manager, you may need to be rescheduled.  If you can not make your appointment on the scheduled day, please notify the case manager and they will reschedule your appointment.
