Mission: To expand and strengthen the capacity for eliminating health disparities through collaboration, education, and partnerships; to champion 100% health access and 0% health disparities.

Primary Purposes of Community Health:

• Make primary healthcare accessible to those in need
• Serve as a focal point for unifying people who have similar interests in eliminating health disparities
• Provide guidance and support for the efforts addressing the health disparity issue
• Maintain diversity among employees and partners

What We've Accomplished:

• Established the importance of Health Disparity Issue in Mecklenburg County
  Identified over 60 partnering organizations and individuals
• Sponsored dental screenings for the uninsured elderly population
  Sponsored community Think Tank Dialogues to identify needs and suggest solutions regarding health disparities
• Sponsored Community Call to Action Forum with former surgeon general, Dr. David Satcher
• Established partnerships for faith-based health screenings; screened over 450 participants for diabetes, hypertension, and cholesterol problems; secured 43 referrals to doctors for medication assistance and follow-up treatment
• Established patient assistance fund for the uninsured with partnering agency
• Coordinated local efforts for "Take Your Loved-One to the Doctor Day"
• Coordinated local efforts for "Uninsured Awareness Day"
