Services In The Home And Community

Community Services provides a variety of programs to ALL people in Mecklenburg County at churches, homes, schools, colleges, child care centers, ambulatory sites, and to the community at large. The overall goal is to improve the quality of life and health for citizens in Mecklenburg County.

The Community Alternatives Program Includes:

• CAP/DA (Disabled Adults)
• CAP/C (Children)

Programs are targeted to high risk populations and aim to address health disparities. Services include, but are not limited to:

• case management, education and outreach
• counseling and testing
• environmental policy development
• home visiting
• disease prevention
