Community Alternative Program/AIDS - CAP/AIDS

Client eligibility:
A potential CAP/AIDS client is one of the following:
- A child up to two years of age who is HIV seropositive.
- A child 2 through 12 years of age who is HIV seropositive and who has at least 2 conditions resulting from the HIV infections.
- An individual who is diagnosed with AIDS.
- An individual living in a private residence and who is at risk of being placed in a nursing facility or lives in a nursing facility and desires to return to a private residence.
- An individual requiring nursing facility care (ICF or SNF) as determined by the Medicaid prior approval process.
- An individual who is eligible for CAP/Medicaid.
- An individual who needs CAP to stay safely at home.
- An individual who can have his or her health, safety, and well being maintained at home within the Medicaid cost limits.
- An individual who desires CAP/AIDS instead of institutional care.
Services:
The Health Department provides detailed assessment and case management for these populations. Consumers are given choices from a network of providers. It makes care at home a real possibility for many who face institutionalization.

- Nursing visits
- Home mobility aids
- Telephone Alert
- DME (Durable Medical Equipment)
- Respite care
- Physical Therapy, Speech Therapy, Occupational Therapy
- Medical supplies
