Community Alternative Program/Disabled Adults - CAP/DA

CAP/DA allows elderly and disabled adults to recceive support services in their home as an alternative to nursing home placement.

Who is eligible?

  • Adults over 18 years of age who are Medicaid eligible and are in need of nursing home level care (intermediate and/or skilled level care) are eligible.
  • Cost of services must be able to be maintained in the CAP budget.
  • Caregiver is required.
  • Safety, health and well-being must be able to be maintained at home.

Intake Procedure:

Apply for Medicaid; call intake coordinator; FL2 form must be completed by doctor.

Cost:

  • There may be a monthly cost to you for CAP/DA services. Your DSS Medicaid eligibility specialist will explain the cost for you.
  • No co-payment for medications and no restrictions on the number of prescriptions and medications if medicaid only (not Medicare qualified)
  • The cost of service to meet your needs cannot exceed the limits set by the Division of Medical Assistance.

Regular Medicaid Services Offered:

  • Durable medical equipment
  • Physical and occupational therapy
  • Speech therapy
  • Nursing (always at least a monthly home visit)

CAP/DA Waiver Services Offered:

  • Case Management (always a service)
  • Adult day health
  • Mobile meals
  • Telephone alert
  • Oral nutrients
  • Home mobility aids
  • Respite care
  • In-Home Aide
