Community Alternative Program/Disabled Adults - CAP/DA

- An alternative to nursing home placement
- Two requirements:
Client eligibility:
Adults over 18 years of age who are Medicaid eligible and are in need of nursing home level care (intermediate and/or skilled level care) are eligible.
Intake Procedure: Apply for Medicaid; call intake coordinator; FL2 form must be completed by doctor.

Must be Medicaid eligible and nursing home level care
- COST: There MAY be a monthly cost to you for CAP/DA services. Your DSS Medicaid eligibility specialist will explain the cost for you.
Among the many services offered are:
Regular Medicaid Services
- Durable medical equipment
- Physical and occupational therapy
- Speech therapy
- Nursing (always at least a monthly home visit)
CAP/DA Waiver Services
- Case Management (always a service)
- Adult day health
- Mobile meals
- Telephone Alert
- Oral nutrients
- Home Mobility Aids
- Respite Care
- In-Home Aide
- No co-payment for medications and no restrictions on the number of prescriptions and medications.
- The health, safety, and well-being of the CAP/DA patient must be assured at all times.
- The cost of service to meet your needs cannot exceed the limits set by the Division of Medical Assistance.
