Community Alternative Program /Children CAP/C

Client eligibility:

Medically fragile children from birth to age 19 who are at risk of being institutionalized without receiving in-home nursing and/or personal care services.

Services:

  • Case management care coordination services provided by a RN / Medical
  • Social Worker Team
  • Personal care services provided by an In-Home Aides
  • Hourly Nursing services
  • Respite Care (in-house and institutional)
  • Home mobility aids such as ramps and/or grab bars for the bathroom
  • Supplies such as nutritional supplements taken by mouth and reusable incontinence undergarments
