Community Alternative Program /Children CAP/C

Drawing of kids playing outsideCAP/C is a program for medically fragile children under age 19 who are at risk for having to live in a nursing home.  By providing in-home
nursing care, case management, and other supports, CAP/C can help
these children stay at home with
their families.

CAP/C parent handbook will answer your questions about the CAP/C program and provide you with information and resources to help you become an informed and active participant in your child’s care.

Who is eligible?

  • Medically fragile children from birth to age 19 who are at risk of being institutionalized without receiving in-home nursing and/or personal care services.
  • Child must be able to be cared for at home in a manner that protects the child's health, safety, and well-being.

Services:

  • Case management care coordination services provided by a nurse
  • Personal care services
  • Hourly nursing services
  • Respite Care (in-house and institutional)
  • Home mobility aids such as ramps and/or grab bars for the bathroom
  • Supplies such as nutritional supplements taken by mouth and reusable incontinence undergarments
