Community Alternative Program /Children CAP/C
|
CAP/C is a program for medically fragile children under age 19 who are at risk for having to live in a nursing home. By providing in-home nursing care, case management, and other supports, CAP/C can help these children stay at home with their families.
CAP/C parent handbook will answer your questions about the CAP/C program and provide you with information and resources to help you become an informed and active participant in your child’s care.
Who is eligible?
Services:
- Case management care coordination services provided by a nurse
- Personal care services
- Hourly nursing services
- Respite Care (in-house and institutional)
- Home mobility aids such as ramps and/or grab bars for the bathroom
- Supplies such as nutritional supplements taken by mouth and reusable incontinence undergarments
