How To Report a Sexually Transmitted Disease

HIV infection, AIDS, all stages of syphilis, and gonorrhea are required to be reported within 24 hours.  Reports can be made by calling 704.432.1742 or faxing reports to 704.336.6200. All reports should include disease, patient name, address, telephone number, date of birth, race, clinic name and telephone number name of person making the report. Laboratory confirmation and treatment should be provided.  The other reportable sexually transmitted diseases that are reportable within 7 days include chlamydia, lymphogranuloma venereum, NGU and PID. A communicable disease report form should be completed and faxed to 704.336.6200 or mailed to:
