Sliding Scale Fee Services

The services below are provided on a sliding scale. Fee is determined by income and family size. Most private insurance is accepted as well as Medicaid, Medicare, and HMO:

• Family planninga
• Breast & cervical cancer screening: 50-64 years
• Dental (children to age 15)
• Pregnancy testing/counseling

What to Bring to Clinic

• Insurance, Medicaid, Medicare and/or HMO card
• Proof of income (W-2, check stub)
• Personal ID (driver's license, birth certificate social security card, etc.)

• Proof of residency for WIC & Dental (electric, gas, telephone bill, etc.)

No proof of ID, residency, or income is required for STD services, but it is important you use the exact same name each visit.  No Mecklenburg County resident will be denied service based on race, color, age, religion, gender, marital status, national origin, or disability.

Free interpreter services are provided.
