How Do I Apply

Simply call the appointment line at 704-336-6500.

Please bring the following to your appointment or to the walk-in clinic:

Nutritionist counseling
  • Identification for each person applying for WIC, including for the parent/guardian of applying for an infant or child.  Examples of identification include Medicaid card, Social Security card, birth certificate, valid driver's license, passport, insurance card, paycheck stub with name printed on it, immunization record, and for infants- the hospital crib card or hospital bracelet is OK.
  • Proof of Residence (where you live).  Examples include current Medicaid card, recent utility bill with applicant's (or parent/guardian in case of child or infant) name and address on it, recent bank statement, valid driver's license, current rental agreement or mortgage statement.
  • Proof of Income.  Examples include current Medicaid card, letter of Food Stamp certification, recent paycheck stub, recent tax forms, letter from employer stating gross income and frequency of pay.
  • Immunization record.  For each infant or child applying to WIC.

Visit Us!   Each person applying for WIC must be physically present, with limited exceptions.  For questions about those exceptions, call the WIC programs secretary at 704-336-6469.
