Local Health Check Coordinators provide education, support and community outreach to high-risk populations and help with Medicaid enrollment into the Health Check & Health Choice Programs.
The Health Check Coordinators (HCCs) follow Medicaid eligible children ages 0-20 to remind parents and care givers to keep appointments and educate them on the importance of well child visits, medical home and emergency room utilization.
