Workers' Compensation

Workers' Compensation Claims staff coordinates adjustment, settlement, and processing of Workers' Compensation Claims for employees of the City of Charlotte, Mecklenburg County, and Charlotte-Mecklenburg Board of Education.

When You Become Injured on the Job...

Call 911 immediately if the injury is serious, or have someone call for you.

Notify your supervisor of all injuries as soon as possible. If you or your supervisor has questions, contact Workers' Compensation - Risk Management Division at (704) 336-3021 for assistance.

Complete an On-The-Job Injury Report. Your supervisor should complete the top portion of a Treatment Authorization Form. Take both completed forms with you to one of the four designated locations of Presbyterian Urgent Care. (Maps)

After treatment, the physician will complete the bottom portion of the Treatment Authorization Form. The form will indicate any work restrictions. Return a copy of the completed form to the Workers' Compensation Section of the Risk Management Division (301 S. McDowell Street, Suite 1100). Remain in contact with your supervisor if you are unable to work due to physician authorized work restrictions.
