General Liability Reporting Form (PDF)
This form should be used when individuals are injured or the property of individuals is damaged due to the alleged negligence of the City of Charlotte, Mecklenburg County, Charlotte-Mecklenburg Schools or its employees. Some examples of when to use this form are.

If an individual, other than an employee while on duty, were to slip, trip or fall and claim injury while on owned property.

If an individual's property was in the care, custody and control of an employee or department and it was lost.

If an employee, while in the process of doing their assigned duties damages an individuals property by any way other than by vehicle.

The ONLY event resulting from the use of a vehicle that should be filed on this form is damage caused by the planned repeated passage of a vehicle on an individual's property. This includes bus park and ride lots.

Accident Reporting Procedures
This form is to be used to report accidents/incidents involving vehicles owned by The City of Charlotte, Mecklenburg County and Charlotte-Mecklenburg Schools. Some examples of when to use this form are.

Anytime that a vehicle is involved in an accident with another vehicle. This includes other owned vehicles as well as citizens vehicles.

If a driver hits a fixed object, (mailbox, fence, etc.) or causes damage to a private drive or yard while operating a vehicle.

If a vehicle is vandalized or is damaged due to windstorm, hail, rain or other natural occurrence.

If a passenger on a vehicle claims to have been injured or suffered some type of property damage or loss.

If a vehicle is stolen or if any equipment that is permanently attached to a vehicle is stolen.
Property Loss Reporting Form (PDF)
This form is to be used to report damage to property other than licensed vehicles owned by The City of Charlotte, Mecklenburg County or Charlotte-Mecklenburg Schools. Mobile equipment such as backhoes and loaders are not considered licensed vehicles and therefore any damage to any mobile equipment will be reported on this form. Some examples of when to use the property damage reporting form are.

If any work related property is stolen from any office building, work site, personal residence or automobile. If property is stolen from an automobile and there are no signs of forced entry/vandalism to the vehicle.

If any property is damaged due to windstorm, hail, rain or other natural occurrence.

If an employee, private individual or vendor causes damage to any property by whatever means.

If an employee leaves a radio or other type of equipment on the bumper of a vehicle and then drives away causing the equipment to become lost or damaged.
