Where is the nearest Congregate Nutrition Site?

You can find the congregate site closest to your home by using the form below.
Follow these steps:
- Enter your house number, street name, and the radius (or distance) from your home that you are willing to travel.
- Press the Submit Address button.
- You will see a black screen that says Select Parcel from the List Below.
- Click on the number (in blue) in the PID Column next to your address.
- a) If your search did not return any sites, the screen will indicate “No Senior Nutrition Sites” or “No Records Found”.
b) If there are sites within the radius/distance of the address you submitted, the list of sites will appear in a separate web page window –we’ll call this the List Screen. The names and telephone numbers of the site coordinators also appear on the list should you need more information about the program or the site.
Behind the list, you will see a map marked with knife & fork symbols indicating the physical location of each site on your list. If you click on the name of a site on the List Screen, it will cause the knife & fork symbol to turn from yellow to green.
- To conduct a new search, close both the list screen and the map screen, then try increasing the radius and press the submit button again in order to find a site closest to you.
