IN-HOME AIDE

Description of Services

This program provides personal care, home management, and respite services to older adults (age 60 +) and adults with disabilities (age 18 +) who are unable to perform these tasks. Services are often provided to remedy and/or prevent neglect, abuse, or exploitation, and unnecessary out-of home placement. These services are designed to provide assistance, not to replace the family in carrying out their responsibilities to individuals needing care and support.  
Individuals, family members/caregivers, or agencies can request In-Home assistance by calling the Customer Information/Referral Center (704-336-3150).  A social worker will complete an assessment.  If it is determined that in-home services are needed, a service plan will be written and a referral sent to the In-Home Aide Program.  An In-Home Aide Contract Administrator will forward your referral to one of our contracted home health agencies. The assigned agency will then contact you to schedule a visit. The agency will assess exactly which tasks are needed for their employee to complete while in your home, and will advise both you and the employee of the assigned day/time that services will be provided.

Mission

  • The mission of In-Home Aide Services is to help the elderly and disabled achieve self-sufficiency, maintain self-sufficiency, and improve their quality of life.

Goal/Objective:  The primary goal of the service is to enable people to remain in their own homes as long as it is safe to do so, thereby preventing premature institutionalization.  The objective is to increase the number of people who are safely maintained in their homes.
