Applying For Benefits


Applications for Work First Family Assistance, Medicaid, and Food & Nutrition Services may be made Monday through Friday between 8:00 am and 5:00 pm at the Wallace H. Kuralt Centre located at 301 Billingsley Road.  Applicants are seen in the order of arrival.

You may also apply for Medicaid and Food & Nutrition Services Monday through Friday between 8:00 am and 5:00 pm at Chapin Hall, located at the old Freedom Mall at 3205 Freedom Drive.
 

Applications for North Carolina Health Choice - a health insurance program for low income children - may be made at the Department of Social Services or by mail.

To request an application form, call 704-336-8422, or an application form is also available here on-line through the North Carolina Division of Medical Assistance.

What Do I Need to Bring With Me to Apply?

When applying for benefits, you DO NOT have to bring any item with you. However, you will be asked to provide information while applying. To speed up the process, it would be helpful for you to bring more than one form of identification (a picture ID if possible) and proof of income with you. This is generally the basic information you will need to get started.


 

Other documents that may be requested during the interview include: 
  • Proof of income received in the last three months
  • Proof of assets owned:
    - checking/savings account statements
    - life insurance policies
    - registration cards/titles for motor vehicles
    - bonds/stock certificates
    - deeds to property
    - proof of outstanding loans on any property
  • Medical bills incurred in the last three months and any old medical bill with an outstanding balance
  • Social Security Cards for family members
  • Birth certificates for family members
  • Immunization records for children
  • Proof of household expenses - rent, utilities, etc.
  • Health/Hospital Insurance Identification cards or policies for family members
