Document provides information regarding the State Endorsement process that we hope you will find helpful as you begin the process. Before beginning the Endorsement process, we strongly encourage providers to read the Policy and Procedure for Endorsement of Providers of Medicaid Reimbursable MH/DD/SA Services. It is very important that you pay close attention to the deadlines for submitting information to the LME because failure to adhere to the timelines in the policy and procedure could result in your agency receiving a Denial of Endorsement.

The renewal process for child residential facilities (.1300, .1700, and .1800) will begin July’2008.  In order to continue to provide residential services your renewal application must be completed and submitted to DHSR no later than October 1, 2008.
