Best Practice Community Teams

 
In 2006, the Mecklenburg County LME initiated the establishment of Best Practice Community Teams for each disability continuum (Adult MH, Adult SA, DD Self-Determination and Child MH/SA). The purpose of the teams is to expand the number of agencies/programs within the Mecklenburg LME community of providers that are providing evidence based services and supports and improve the quality of treatment, services and supports through the adoption of best practices.

Membership:

Consumers and family members, representatives from advocacy agencies, providers, community partners and interested stakeholders.

Tasks/Activities:

  1. review current evidence based practice models & emerging best practices; 
  2. review current services/continuums of care; 
  3. determine if & how current services match up to evidence based practice models; 
  4. assess and identify gaps in service; and,
  5. develop strategies to implement best practice models in the community. Recommended

Actions:

Adoption of specific evidence-based practices within each continuum, method for implementation & development (ex. through RFP/RFI process, grant opportunity), reduction/elimination of services identified as not being best practice models, identification of quality improvement opportunities.

Providers who intend to practice in an environment where knowledge and utilization of best practices is the standard, participation is an imperative. The four community teams meet monthly as follows:

Adult MH Best Practices Community Team - Meets 1st Friday, 10:00 - 12:00 at Sam Billings Center, 429 Billingsley Rd.
Facilitator: Ellis Fields, Mental Health Association

Adult SA Best Practices Community Team - Meets 2nd Wednesday, 3:00 - 4:30 at Sam Billings Center, 429 Billingsley Rd. Facilitator: Dennis Knasel, LME.

DD Self-Determination Best Practices Community Team - Meets 1st Monday, 1:00 - 3:00 at VR Office, 5501 Executive Center Dr. Facilitator: Dave Dougherty, Southminister
Service Gaps/Best Practices Committee - Meets 2nd Friday, 8:30 - 10:00 at Alexander Youth Network, Thermal Road.
Facilitator: Libby Safrit, Teen Health Connection
